0b54f3c087 Remove gif demuxer, the code contains a gif decoder and lzw decoder neither 
belongs in a demuxer, furthermore the code might be exploitable.

eb2cd99c73 mlpdec: Check for bits left before each read of End-of-Stream indicator and 
Substream parity check independently. This allows decoding of streams that
have EOS but don't have Substream parity check.

d08e3e91af Prevent users from setting -vol and -acodec copy together. 
They are incompatible since the frames are not decoded, so they can't be preprocessed.
