Dr. Stephen Henson 6ec8e63af6 Port BN_MONT_CTX_set_locked() from stable branch.

The function rsa_eay_mont_helper() has been removed because it is no longer
needed after this change.

The stuff in here is based on patches supplied to me by
Steven Schoch <schoch@sheba.arc.nasa.gov> to do DSS.
I have since modified a them a little but a debt of gratitude
is due for doing the initial work.
