Make the definition of HMAC_CTX opaque
This moves the definition to crypto/hmac/hmac_lcl.h. Constructor and destructor added, and the typedef moved to include/openssl/ossl_typ.h. Reviewed-by: Rich Salz <rsalz@openssl.org>
