There were numerous small issues with test cases:
- some lacked the right source file header
- some were not portable at all
- some were using internal libzmq APIs (headers)
Solution: fixed and cleaned up.

This commit addresses the following warnings reported on gcc 5.2.1. In
the future, this will help reduce the "noise" and help catch warnings
revealing a serious problem.
