This file defines a single, huge function, MPV_motion(), which
although being declared inline is not actually inlined by the
compiler (for good reason). There is thus no sense in defining
this function in a header file, resulting in multiple copies of
it in the final library.

This fixes compilation failures related to START_TIMER/STOP_TIMER macros and
-Werror=declaration-after-statement. START_TIMER declares variables and thus
may not be placed after statements outside of a new block.

The low 32 bits of a multiplication are the same for signed
and unsigned operands. Casting to unsigned before multiplying
is thus equivalent while avoiding signed overflow, which is
undefined by the C99 standard.

These tables contain only a 1-bit flag each. Combining them reduces
the data size and saves some instructions in the block decode loop.

This allows for some simplifications like removing some outer loops
and gives much better performance for thread_count > number of idle CPUs.

This is mainly to avoid test failures due to implementation-defined
behaviour of qsort when elements are equal, giving different results
for each of FreeBSD, Linux/glibc and Solaris.
In addition it is about 35 % faster, effect on overall speed is minimal
though (< 2%).
Regression tests are unchanged (i.e. identical to Linux/glibc).
