7195731522 Remove *_codec_id from AVFormatParameters with the next major version bump. 
AVFormatParameters is never available when needed and is an annoying mess.

6d96a9b94d elaborate a little on AVStream.start_time, it seems the comment is not clear 
enough, not that the clearer NEVER comment in AVFormatContext.start_time stoped
people from posting stupid patches
