After much discussion and back-and-forth, we reached the conclusion
that matroska uses convergence_duration for subtitle duration because
a 32bit value isn't large enough to store the duration if sub-micro-second
timebases are used. Matroska may not be the only one that supports these
timebases, but it's certainly the only one that ffmpeg attempts to support
in this way.
The long term solution that we seemed to reach was that if we encounter
a matroska file with a sub-micro-second timebase, we should internally
scale it up to at least micro-second, and then duration can be used
normally. This suggests that on the encode side, we should not allow
generation of files with sub-micro-second timebases, but that's a separate
issue.
That being a non-trivial change, and the subtitle interoperability breakage
being very real, I'm re-submitting this small change for consideration.
In this diff, we make sure that duration is populated by the matroska
demuxer, and that convergence_duration is respected in matroskaenc and
srtenc, but that duration is used otherwise. This ends up being a strict
improvement - pipelines that use convergence duration are unchanged, and
ones that are currently broken due to the duration mismatch will start
working - except for the ones with the extreme timebases, but those were
already broken.

While not explicitly stated in the specs, the original author
has stated that S_TEXT/UTF-8 is expected to be text using Subrip
markup, but without Subrip in-band timing.
So, now that we have a decoder that conforms to this expectation,
let's use it.
Note that this change will impact tools that use libavformat. If
they expect srt subtitles to have CODEC_ID_TEXT, they must be
adjusted to expect CODEC_ID_SUBRIP. The actual content is, obviously,
unchanged.

According to its description, it is supposed to be the LCM of all the
frame durations. The usability of such a thing is vanishingly small,
especially since we cannot determine it with any amount of reliability.
Therefore get rid of it after the next bump.
Replace it with the average framerate where it makes sense.
FATE results for the wtv and xmv demux tests change. In the wtv case
this is caused by the file being corrupted (or possibly badly cut) and
containing invalid timestamps. This results in lavf estimating the
framerate wrong and making up wrong frame durations.
In the xmv case the file contains pts jumps, so again the estimated
framerate is far from anything sane and lavf again makes up different
frame durations.
In some other tests lavf starts making up frame durations from different
frame.

The new incremental parser doesn't always clear prev_pkt,
however the packet queue is cleared when seeking. Which leads
to a use-after-free.
Verified using Valgrind.

Reduces the amount of upfront data required for cluster parsing
thus decreasing latency on seek and startup.
The change in the seek-lavf_mkv FATE test is due to incremental
parsing no longer reading as much data as the old parser and
thus not having that additional data to generate index entries
based on keyframes. Index entries are added correctly as the
file is parsed.
All FATE tests pass and Chrome has been using this patch for ~6
months without issue.
Currently incremental parsing is not supported for files with
SSA tracks since they require merging packets between clusters.
In this case the code falls back to non-incremental parsing.

If a video track specifies a zero frame rate (invalid but occurs),
this results in a division by zero and subsequent undefined conversion
to integer. Setting the default duration from the frame rate only
if the latter is greater than zero avoids such problems.

This replaces the matroskadec one with the same name.
The advantage is not only easier reuse in other demuxers
but also that we can make the decisions after the parser.
This fixes seeking in files that mark the keyframes incorrectly,
for example the file in track ticket #1003.
The matroska variable is still kept to be able to complain
about such broken files.

This prevents certain tags with a default value assigned to them (as per
the EBML syntax elements) from ever being assigned a NULL value. Other
parts of the code rely on these being non-NULL (i.e. they don't check for
NULL before e.g. using the string in strcmp() or similar), and thus in
effect this prevents crashes when reading of such specific tags fails,
either because of low memory or because of targeted file corruption.

The parser was fixed so this workaround should no longer
be necessary.
This allows using stream-copy to fix files with keyframes

In particular, detect when the index is obviously broken.
This fixes the worst symptoms of trac issue #958 and makes
sense to allow seeking in files without index.

The existing functions defined in intfloat_readwrite.[ch] are
both slow and incorrect (infinities are not handled).
This introduces a new header with fast, inline conversion
functions using direct union punning assuming an IEEE-754
system, an assumption already made throughout the code.
The one use of Intel/Motorola extended 80-bit format is
replaced by simpler code sufficient under the present
constraints (positive normal values).
The old functions are marked deprecated and retained for
compatibility.

This change fixes a bug where seeking doesn't work properly for
matroska files that have the CUES element before the first cluster.
This bug was accidentally introduced a few months ago by my deferred CUES
loading patch<http://git.videolan.org/?p=ffmpeg.git;a=commit;h=31ad14c21e0735387ba8082c6e3436241f7ccfc8>
.
When the CUES element appears before the first cluster in the file, the data
is parsed and placed in matroska->index but that data is never added to the
seek index. Currently the transfer from matroska->index to the seek index
only happens when matroska_parse_cues() is called.
Matroska_parse_cues() only gets called on a seek if cues_parsing_deferred is
set. Cues_parsing_deferred only gets set if parsing the CUES requires
seeking past the first cluster. There is no code to handle the case where
CUES is before the first cluster.
This fix essentially restores the matroska->index processing that was
happening at the end of matroska_read_header() before I made my CUES
deferral change. In the case where CUES is before the first
cluster, matroska->index will have data and the seek index will be updated.
In the case where CUES is later in the file, matroska->index will be empty
and cues_parsing_deferred will be set so loading will happen later.

EBML_STOP leaves matroska->current_id set. Then matroska_read_seek changes
the stream position without resetting current_id. The next
matroska_parse_cluster fails due to calculation of incorrect pos. So clear
current_id when avio_seek happens in matroska_read_seek.

Make AVIO_FLAG_ access constants work as flags, and in particular fix
the behavior of functions (such as avio_check()) which expect them to
be flags rather than modes.

ff_get_wav_header is reading data from a WAVE file and then uses it
(without validation) to malloc a buffer. It then proceeded to read
data into the buffer, without verifying that the allocation succeeded.
To address this, change ff_get_wav_header to return an error if
allocation failed, and adapted all calling code to handle that error.

When a normal Block is parsed, duration is initialized to
AV_NOPTS_VALUE. If it is not changed, then the track's default
duration is used. But for SimpleBlock, duration is initialized to
0 instead of AV_NOPTS_VALUE. This is due to the difference in how
EBML_NEST vs EBML_PASS are processed. Setting duration to 0 leads
eventually to wrongly estimate the frame duration in util.c

it.
This has the advantage that it is easy to add new limits and makes
it less likely to forget to add checks and also avoids issues like
bits of the length value above the first 32 being ignored because
the parsing functions only takes an int.
