lavc: replace AVCodecContext.encode with subtitle-specific callback
AVCodecContext.encode is currently used only for subtitles, encode2 is
used for audio and video.

Add ff_ prefix to data symbols of encoders, decoders, hwaccel, parsers, bsf.
None of these symbols should be accessed directly, so declare them as
hidden.
